When the golf club head comes too much outside of your hands, you've lost your swing plane line and problems can occur.

If you're leaning over too much in your setup, there can be a tendency to swing the club back too far to the outside. Keep a good posture with good angles and then simply swing and turn while maintaining those same angles. This promotes a more on plane golf swing as well.
